Request for Letter of Good Standing

Important! Only Graduate Student Advisors can initiate requests for Letters of Good Standing.  Students should not use transient forms from the Registrar's Office.  Advisors - please complete all sections.  (Please note that graduate students who seek transfer credit should consult with their advisor to see if a course is appropriate for transfer credit, and to determine how many hours can be transferred based upon the policies of a given program.)

Include first, middle, and last names.

Include street address, city, state, and zip code.
Include course prefix, number, title, and credit hours for all courses.
When will the course(s) be taken?
Include a brief justification for the request.
