Request Accommodations in a Workplace Environment (e.g., practicum, internship, externship, clinical, student teaching)

As a result of the unique nature of workplace environments, the same methods used to remove barriers in the classroom may not be effective or appropriate in a particular placement. In order to determine 1.) program requirements, 2.) essential functions of a workplace assignment, 3.) potential areas of inaccessibility, and 4.) appropriate accommodations unique to each assignment, an individualized assessment must be completed by appropriate parties (e.g., student, placement coordinator, placement supervisor, Access Office team member). This process should be initiated as soon as possible by completing the form below. Students are encouraged to do so prior to placement (according to the academic program timeline) since a “good fit” may reduce the need for accommodations.

Please provide the following information in order to begin the process to arrange an accessible workplace experience. Once this form is submitted, the Access Office will contact the appropriate faculty member (usually the placement coordinator) within 10 business days to discuss the details related to the academic program and determine a timeline for assessment and implementation.
