Withdrawal Before Midterm (Course withdrawal):

  1. *Pick up a Course/College Withdrawal Form from the Registrar's office.
  2. Complete sections A & B.
  3. Obtain instructor's signature. The instructor will assign a "W" at the time of withdrawal.
  4. Return the completed form to the Registrar within 5 business days.

*If you are dropping a course to change sections or add a course, you must also use a Late Registration and Drop/Add Form and a Registration Appeal Form. 

A withdrawal is not official until received and processed by the Office of the Registrar.

Withdrawal After Midterm:

  1. *Pick up a Course/College Withdrawal Form from the Registrar's office.
  2. Complete sections A, B & C.
  3. Obtain the instructor's signature. The instructor will assign a "W" or "WF" at the time of withdrawal.
  4. Obtain the Advisor's, Dean's, and VP of Academic Affairs signatures.
  5. Return the form to the Registrar.

*If you are dropping a course to change sections or add a course, you must also use Late Registration and Drop/Add Form.

A withdrawal is not official until received and processed by the Office of the Registrar.

Withdrawal From College (all courses):

  1. *Pick up a Course/College Withdrawal Form from the Registrar's office.
  2. Complete sections A & B.
  3. Have the Office of Student Affairs complete Section D.
  4. Obtain the Office of Student Affairs signature.
  5. Return the completed form to the Registrar.

*If you are dropping a course to change sections or add a course, you must also use Late Registration and Drop/Add Form.
