Reception

A Student Health Center receptionist greets students upon arrival at Student Health.

All students register or sign in on the provided form, giving up-to-date information. For those students having their first visit at Student Health, additional paperwork completion is required.  These forms may be downloaded, printed, completed in advance, and brought with the student at their first clinic visit.

Registration Record & Receipt of Notice of Privacy Practices

Each student will have an individual chart.  Student Health Center follows a confidential filing system adhering to HIPAA guidelines.
